Request approval to vary and extend the contract for the provision of Integrated Health Improvement/ Wellness Service. The variation and extension is required to allow sufficient time for transition and mobilisation of the new service, post procurement. The Integrated Health Improvement/ Wellness Service known as One You Haringey (OYH) aims to provide identification of risk factors, provision of health improvement advice and interventions relating to factors and conditions associated with ill health and its determinants, for improved population and individual level outcomes and to reduce health inequalities and promote a healthier lifestyle for all. The extension value of the contract is £229,167 for a period of five (5) months, commencing from 1 September 2023 to 31st January 2024. The aggregated total value of the contract will be £687,500 including the above proposed variation and extension.??
